How the process works is:
- We will speak to you on the phone about your needs and concerns before your first session
- You will complete initial paperwork online through Simple Practice that is linked right through our website to be completed before your first session
- You will then receive an email with your selected and confirmed appointment date and time from Simple Practice
Session Format:
- The first session will be about getting to know you and developing your treatment plan by making goals
- Following sessions will be about 45-60 minutes long with a few minutes at the end to review our treatment plan progress and plan your next session.
Privacy / Confidentiality
*Privacy and confidentiality are extremely important to us and a guiding ethic for our practice. What you share in a session is confidential, except when required by law. Such instances are: suspected child abuse, elder abuse, or neglect, imminent harm to client or other, reporting information required in court proceedings or by a client’s insurance company, or other relevant agencies, or defending claims against Clear Mindset Counseling Center, Inc.
*Email is not secure for confidential information so we ask that you speak to us directly for intricate issues and keep identifying information out of emails (name, date of birth, and so on)
*If another party such as your doctor, school, employer, or other family member requests information, we will not disclose information unless a Release of Information is completed by you and signed.
Client Rights
You have the following rights as a person receiving services for mental health:
- To expect that as mental health providers (social workers) we have met the expectations for education and training required by state law and the State Board of Clinical Social Workers
- To request records and information from The Board of Clinical Social Workers pertaining to our credentials
- To file a complaint with this agency or The Board
- To know fees in advance of receiving services
- To have confidentiality except in specific circumstances outlined in our Privacy and Confidentiality section
- To be treated fairy, respectfully, and free of discrimination based on: race, gender, ethnicity, religion, or any other category
